Which Squarespace plans allow Code Injection?

It is available on the Core, Plus, and Advanced plans, plus legacy Business and Commerce plans. The Personal plan does not include Code Injection.

Do I have to add a code block to each page?

No. The Footer under Code Injection applies your snippet site-wide, so it is a one-time setup.
